Juanita McIntosh 85 passed away October 16, 2012.
She was a resident of Indianapolis.
She was born August 18, 1927 in Paintsville, KY to the late Albert and Janie (Pack) Mollett.
Survivors: sons, Michael, Leonard, and Vincent McIntosh; daughters, Carolyn Smith, Charlotte Evans, Vickie Ross and Andrea McIntosh; brothers, Noah, Paul and Junior Mollett; sister, Salva Smith; nineteen grandchildren and several great and great great grandchildren.
Funeral services 2:00 p.m. Monday, October 22, 2012 at Forest Lawn Funeral Home. Visitation from 4:00 PM - 8:00 PM Sunday at the funeral home. Entombment will follow in Forest Lawn Memory Gardens.
